Placing an order for cremation you will need:
- an authorization form filled by the nearest relative or person authorized to organize cremation: [download here]
- an abridged copy of Certificate of Death issued by the Registry Office or its certified photocopy bearing the stamp of a funeral home and true copy clause.
- in the case of the cremation of human remains, a permission of the State Sanitary Inspectorate is required
